Sebaceous Filaments vs Blackheads: Why the Dots on Your Nose Keep Coming Back

Not every dot on your nose is a blackhead. Here's how sebaceous filaments differ, why they come back after extraction, and what actually helps.

You squeeze, you strip, you scrub, and a few days later the tiny dots on your nose are back. Sound familiar? Here's the thing: many of those dots aren't blackheads at all. They're sebaceous filaments, a completely normal part of your skin. Once you know which is which, you can stop fighting your pores and focus on what actually makes a difference.

    What are sebaceous filaments?

Every pore on your face is connected to an oil gland. That gland produces sebum, your skin's natural oil, which mixes with skin cells and travels up to the surface. Sebaceous filaments are simply that oil-and-cell mixture lining the pore, showing up as faint grey or yellowish dots, usually across the nose.

They're not acne, and they're not dirt. Everyone has them. They're just more visible on oilier skin and in larger pores.

    How is a blackhead different?

A blackhead is a pore that's actually clogged. Oil and dead skin cells form a plug, and the top of that plug darkens when it meets the air. That's why blackheads look darker and more defined than sebaceous filaments. (And no, the dark colour isn't dirt either, so scrubbing harder won't fix it.)

Blackheads are a mild form of acne, which means they can respond to acne care.

    How to tell them apart

    What it is

    Sebaceous filaments
Normal oil and skin cells lining the pore
    Blackheads
A pore blocked by a plug

    How it looks

    Sebaceous filaments
Small, even, grey or yellowish dots, often across the nose
    Blackheads
    Darker, more defined dots

    Part of acne?

    Sebaceous filaments
    No, a normal skin feature
    Blackheads
Yes, a mild form of acne

    After extraction

    Sebaceous filaments
Refill as your skin keeps making oil
    Blackheads
Can come back if pores keep clogging

You can have both at the same time, and they can look alike, so if you're unsure, your doctor can take a look.

Why do the dots keep coming back?

Extraction only removes what's in the pore right now. Your oil glands don't stop working, so sebaceous filaments naturally refill. Blackheads can also return if whatever's causing the clogging hasn't been addressed.

That's why squeezing again isn't the answer. Repeated squeezing and pore strips can irritate the skin, stretch the pore and even lead to inflammation or scarring, without stopping the dots from returning.

    What actually helps

For sebaceous filaments, the goal is to keep them less noticeable, not to empty your pores for good. Gentle cleansing, oil control and avoiding harsh scrubs go a long way.

For blackheads, your doctor may suggest topical acne treatment to help stop pores from clogging in the first place. And if your pores are congested or look enlarged, in-clinic treatment can help refine texture and regulate oil over time. At Reborn, our Hybrid Perfecting Laser is one option your doctor may suggest to improve pore visibility and smooth skin texture.

When to get your skin checked

See your doctor if the area becomes red, swollen or painful, if you're also getting regular breakouts, or if some dots look different from the rest. Let them know what products you use and whether you've had extractions at home or at a salon.

    Frequently asked questions

    Are sebaceous filaments blackheads?

No. Sebaceous filaments are normal pore contents, while blackheads are clogged pores. They can look similar, but they're different things.

Can I get rid of sebaceous filaments permanently?

No, because your skin keeps producing oil. But you can make them less noticeable with the right skincare and, if needed, in-clinic treatment.

    Do pore strips work?

They pull out the top of the filament for a short while, but the pore refills. Frequent use can also irritate your skin.

Will extracting blackheads stop them coming back?

Not on its own. Extraction clears what's there, but preventing new clogs usually needs ongoing care.
